On your initial visit there are several items
of information you need to bring:
- Vital medical records.
- A list of all medications you
are currently taking.
- X-rays or reports.
- Current medical insurance
information.
- A referral, if required by your
insurance carrier.

To serve our patients efficiently, we see patients by appointment only. We
make every effort to stay on schedule, however, emergencies do arise that
require our immediate attention. If possible, we will notify you of such a delay
to minimize any inconvenience. |
